Admissions


In an effort to ensure that Lily Pad Living is the right fit for you, we ask that you complete our admissions process:

Housing Application Referral Form

  1. Complete and submit your application. Using the application linked here, we ask that you complete the information to the best of your ability. Once you have completed all applicable fields in the application, please scan and email it directly to Stephanie@LilyPadLiving.com

  2. Wait for us to review your application. Please allow 24-48 hours for the information review process to be completed. If you have any questions about your application in the meantime, feel free to call (707)384-4622 to speak to our Facility Director (or to be directed to another member of our team). 

  3. Once your information has been processed, a team member will contact you. After your information has been fully reviewed, one of our team members will contact you by phone. During this phone call, we will proceed with a quick interview to make sure our transitional living program is a good fit for you. Once that is confirmed, we will reserve your spot and schedule your enrollment at the location of your choice. Please allow 24 hours for a returned phone call during calendar business days.

NOTE: If you are a service provider who would like to refer one of our clients to Lily Pad Living, we ask that you please complete our Service Provider Referral Form. This can be completed, scanned, and emailed directly to Stephanie@LilyPadLiving.com.

We also ask that you have your client fill out our Housing Application. Your client can scan and email the application to Stephanie@LilyPadLiving.com- OR, you as the service provider can also submit it to us.
